i am an ohip patient
All OHIP patients are required to present a valid health card before each visit with a physician.
Failure to present a valid OHIP card will result in a $30 CAD fee which may be reimbursed upon proof of a valid health card.

i am not an ohip patient
Patients without an Ontario Health Card, private patients, residents of other provinces or countries, visitors to Canada, and newcomers are welcome to visit Restore Medical Clinics and receive care; however, they are responsible for the costs associated with their visit (some exclusions apply).
Uhip patients
Individuals covered by a University Health Insurance Plan (UHIP) do not have to pay for their visit. We ask that you bring your UHIP card with you for your first visit. Thereafter, we can keep a copy on file for future visits.
This does not apply to individuals with a College Health Insurance Plan (CHIP).
refugee patients
Individuals with their Refugee Protection Claimant Documents are covered by BlueCross and do not have to pay for their visit. We ask that you bring your document with you for your first visit. Thereafter, we can keep a copy on file for future visits.
out-of-province patients
Patients residing outside the province of Ontario are responsible for the costs associated with their physician visits.
Once payment is confirmed we provide a receipt that can be submitted to the appropriate governing body for reimbursement.
For Quebec patients we also provide a RAMQ form to be submitted to the Régie de l'assurance maladie du Québec.
